Upon booking your food intolerance test with Amy, you will receive your confirmation by email and a second email containing a questionnaire.

In order for the appointment to run smoothly and to schedule, please fill in the food intolerance symptoms questionnaire that I will send to you following your booking email. please send this back to me 48 hours before your test.

If you are unable to fill the form in online please print off and complete by hand, take a photo and attach image and send to my email. Thank you.
